Responsible for leading the company's end-to-end patient engagement strategy and execution as well as operations across the Palliative Care at Home Program. This role utilizes key engagement related systems, implements patient engagement offerings for new and existing palliative care programs, drives toward key performance related metrics, and manages a team performing engagement activities. This role creates meaningful outreach and connections for patients to consider care offered by the company and works closely with external partners, including joint venture health systems as well as payors. Responsible for working internally with clinicians and other home-based care team members to ensure patient visits are scheduled as consistent with the clinical model visit cadence and frequency.


Responsibilities
  • Oversees the patient engagement strategy and execution for the Palliative Care at Home Program.
  • Works across health plan and health system partners to develop strategies that increase outreach connectivity and conversion for Palliative Care at Home program.
  • Leads a team of Engagement Specialists that will speak with patients and caregivers across all Palliative Care at Home programs with a goal of opting into the program.
  • Builds the engagement program to include expanding current palliative care programs and supporting the implementation of new programs with strategic hiring of additional Engagement Specialists and scheduling personnel.
  • Collaborates with the company's marketing leadership and partners to create patient and provider facing content.
  • Works in partnership with the palliative care clinical leadership to enable person-centered engagement and maintain positive relationships with key stakeholders.
  • Reports engagement outcomes to hospital partners, health plans and Contessa senior leadership.
  • Responsible for utilizing data-driven decision-making and culture to increase patient satisfaction and company performance.
  • Directs the development of goals, objectives, policies, and performance metrics for the engagement team.
  • Creates processes and scalable systems to achieve performance targets across all palliative care programs and with partners across multiple markets.
  • Partners with Data Analytics in the development, collection, analysis and tracking of data trends to evaluate the effectiveness of patient engagement efforts.
  • Monitors and evaluates employee performance. Holds employees accountable for the attainment of defined goals and objectives. Executes effective performance redirection when needed. Develops employees through coaching, mentoring, and formal/on the job training and development opportunities.
  • Ensures compliance with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.
  • Performs other related duties as assigned.

Qualifications


Required

  • Bachelor's degree in health & Human Services, Education, Business, Health Care Administration, or related field; or eight (8+) years of leadership experience in healthcare in lieu of degree requirement.
  • Eight (8+) years of experience in healthcare community outreach, engagement, and/or patient experience and progressive leadership and five (5+) years of management responsibility.
  • Experience building and using a client engagement system.
  • Experience working with health plan and/or health system partners.
  • Experience managing a growing engagement team, including developing and executing new and innovative patient engagement strategies with the team as engagement targets scale.
